The real wearable blood pressure startup race currently has six credible names: Sky Labs, Hilo, Biobeat, LiveMetric, Vital Signals and Valencell.

We define this market narrowly. The device must be worn on the wrist, finger or chest and estimate systolic and diastolic blood pressure without repeatedly inflating a conventional arm cuff. We include products already used by patients, regulated devices entering commercialization and late-stage products close enough to launch that they could change the ranking.

We exclude ordinary wrist cuffs, smartphone-camera apps and university prototypes. We also exclude Samsung, Huawei, Apple, Omron and Whoop because they are established companies rather than startups. Oura remains outside the comparison because it does not currently provide blood pressure readings.

Nanowear is a borderline case. Its SimpleSense platform has explored cuffless blood pressure monitoring, but Nanowear is building a broader clinical monitoring system rather than competing mainly through a blood pressure wearable.

Is one wearable blood pressure startup clearly ahead today?

Sky Labs has passed the point where a cuffless blood pressure wearable is merely an interesting device. Its CART BP pro ring has received Korean medical approval, entered an established reimbursement pathway and accumulated more than 250,000 prescriptions across over 1,700 medical institutions. That is the strongest evidence in the market that doctors are repeatedly using the product with real patients.

Hilo has built the largest visible consumer business. The company had sold more than 120,000 devices before beginning its latest U.S. expansion, with approvals or certifications across Europe, Canada, Australia, Saudi Arabia and the United States. Hilo also has far more capital than Sky Labs and a longer history of people wearing its product at home.

The remaining gap is clinical depth. Hilo's current U.S. Hilo Core package is presented as a wellness product, even though the company separately received FDA clearance for its G0 system. Sky Labs has made its regulated medical product central to its commercial model rather than running medical and wellness propositions side by side.

Biobeat belongs in the leading group because its chest patch has multiple FDA clearances and a clear use case in 24-hour ambulatory monitoring. Still, it has not published patient or clinic volumes comparable with the numbers disclosed by Sky Labs and Hilo.

More than 250,000 cumulative prescriptions give Sky Labs a level of proven demand that no other cuffless blood pressure startup has publicly matched. Its presence across over 1,700 medical institutions also shows that adoption is spread across a network rather than concentrated in one research hospital or commercial pilot.

Hilo is the clear number two. Its more than 120,000 device sales represent genuine consumer purchases, and that already separates Hilo from startups that have regulatory clearance but disclose little evidence of recurring demand.

The two numbers cannot be compared perfectly. One Hilo device can stay with a customer for years, while a patient may receive more than one Sky Labs prescription. The company is widening its business from individual monitoring toward real-world evidence and drug-development data.

Vital Signals has made the fastest entrance. The company raised more than $15 million, opened preorders for a $399 Signal Ring and says initial deliveries will begin later this year. Its pitch is unusually clean: blood pressure from a ring, no calibration cuff and no subscription.

That momentum is still based on promises. Vital Signals must manufacture multiple ring sizes, ship on time, publish full validation evidence and show that calibration-free readings remain stable outside controlled testing. Strong preorder interest would not answer any of those questions.

Hilo is also moving quickly through its U.S. launch. The company now has an active American storefront and is marketing frequent blood pressure tracking through Hilo Core. The catch is that Hilo Core is described as a wellness product, while the separately cleared G0 system comes with a narrower FDA indication.

Biobeat has lately added both money and distribution support. Its $50 million Series B and partnership with Kestra Medical Technologies give it a credible path into U.S. cardiology. We still need patient volumes, clinic counts or completed tests before calling that commercial acceleration.

Which wearable blood pressure startup has the most mature product?

Sky Labs has the most mature medical product, while Hilo has the most mature everyday consumer experience.

CART BP pro already fits into a real hypertension workflow. A doctor can prescribe the ring, the patient can wear it during normal activities and sleep, and the resulting report can help identify nighttime hypertension, morning surges, masked hypertension or changes after medication.

Hilo has the stronger history of long-term home use. Its European wristband has been sold since 2021 and is designed to collect readings quietly throughout the day and night. Hilo has therefore had several years to improve its app, calibration process, battery management and customer support.

The U.S. product story is more complicated. The FDA-cleared G0 system requires regular cuff calibration and has a defined spot-checking indication. Hilo Core uses the same broader technology family but is currently marketed as a wellness system rather than a medical device. Consumers may not notice that distinction, but doctors and regulators will.

Biobeat is highly mature for one specific job: replacing an uncomfortable inflatable ambulatory cuff during a prescribed 24-hour test. Its disposable chest patch makes less sense as a permanent lifestyle wearable, although permanent wear is unnecessary for the clinical task it targets.

LiveMetric received FDA clearance in 2022 for clinical use in adults. That remains a valuable asset, although the company has not followed it with the geographic expansion or visible adoption achieved by the leaders.

Vital Signals currently sells Signal Ring as a consumer preorder. Valencell has spent years developing calibration-free optical technology, but its proposed fingertip monitor has not reached a verified FDA clearance.

Hilo has the cleanest published long-term validation, while Sky Labs has the larger development dataset and stronger proof that clinicians will use the technology.

Hilo's most useful published study followed 91 adults for one month under an extended ISO 81060-2 protocol. The wristband stayed within the protocol's accuracy requirements, including among participants with different skin pigmentation and levels of forearm hair. A month is still short, but it addresses calibration stability better than a single laboratory session.

Sky Labs built its algorithm from 320,535 arterial-line blood pressure samples collected from 4,185 people. That is the largest disclosed development dataset among the startups we reviewed. Smaller device studies have also reported mean systolic and diastolic differences within accepted numerical limits.

The studies answer different questions. Hilo provides clearer published evidence that a finished wearable can remain stable after calibration. Sky Labs provides stronger evidence of dataset depth and far more evidence that doctors consider the finished product usable.

LiveMetric published an invasive-reference study involving 34 patients. Its correlations and average errors looked promising, although such a small cohort cannot settle performance across different ages, skin tones, movements and cardiovascular conditions.

Vital Signals says Signal Ring met ISO criteria in a 451-person multicenter study. That could become one of the strongest studies in the category. For now, only the company's summary is available, so we cannot inspect the complete protocol, error distribution or failure rate.

Biobeat has several clinical publications and FDA-reviewed submissions, but its evidence is spread across different cardiovascular measurements and use cases. A direct modern comparison with conventional ambulatory blood pressure monitoring would make its position easier to judge.

Are wearable blood pressure monitors accurate enough for medical decisions?

Wearable blood pressure monitors are useful for tracking patterns today, but they are not ready to replace validated arm cuffs for diagnosis or medication changes across the general population.

A doctor needs to know how often the device produces a large error for one patient, especially when treatment decisions depend on differences of only a few millimetres of mercury.

The American Heart Association has therefore stopped short of recommending cuffless devices as replacements for validated cuffs in clinical care. Recent FDA guidance follows the same logic. Manufacturers are expected to test initial accuracy, stability over time and the ability to follow genuine blood pressure changes rather than merely reproducing values close to calibration.

Hilo and Biobeat reduce some uncertainty by using cuff calibration, although that weakens the promise of removing cuffs completely. Sky Labs has earned greater clinical trust within its approved use, but broader independent testing would still help. Signal Ring could remove one of the category's biggest inconveniences if its calibration-free design works consistently, yet company claims and shipped performance are two different levels of evidence.

For now, these products make the most sense for observing trends, nighttime patterns and changes between conventional measurements. A validated upper-arm cuff remains the safer reference when a reading could alter treatment.

The trade-off is calibration. Hilo users still need a companion cuff, and the exact calibration schedule varies between product versions and regulatory markets. The system also requires an active membership in its current commercial packages.

Signal Ring removes almost every visible inconvenience from the pitch. The $399 ring promises background blood pressure tracking, no calibration and no subscription. It is also easier to wear alongside a watch than another wristband would be.

None of that has been tested by a large customer base yet. A ring can perform badly when the size is slightly wrong, and fingers experience temperature and blood-flow changes that can affect optical measurements. Sky Labs has deliberately used established healthcare companies rather than building every national sales force itself. That approach helped it expand in Korea and gives it a realistic route into Japan.

Biobeat's patch creates both an opportunity and a burden. Otsuka Pharmaceutical's planned Japanese commercialization of CART BP pro is the clearest example. Otsuka already works with doctors treating cardiovascular disease and can place the ring inside an existing medical sales channel. That relationship is far more useful than a general partnership announcement with no defined buyer or distribution role.

Sky Labs previously worked with Daewoong Pharmaceutical in Korea and Omron Healthcare on Japanese consumer distribution. The repeated pattern is easy to see: Sky Labs develops the sensing and data platform, while established healthcare companies provide local access.

Biobeat's Kestra Medical Technologies partnership is also strategically useful. Kestra sells wearable cardiac technology and works with high-risk cardiovascular patients, giving Biobeat a natural group of clinicians and patients to target. The partnership includes investment, licensing and product-development elements rather than a vague commitment to explore opportunities.

Hilo's biggest customer asset is its direct consumer base. What remains less visible is a deep enterprise contract with a major insurer, health system or employer that pays for large-scale ongoing use.

LiveMetric once described health systems, insurers and self-insured employers as its target customers. Public evidence that those targets became large recurring contracts has remained scarce. Hilo's higher spending partly reflects its direct-to-consumer strategy. Advertising, fulfilment, support and international subscriptions require more capital than selling through pharmaceutical partners in one home market. Hilo is the only current rival that could overtake Sky Labs quickly, although Signal Ring has the largest upside if its calibration-free claims survive real use.

Hilo's next decisive numbers will be active U.S. customers, renewal rates and accepted readings per user. Large consumer sales would become far more valuable if people continue paying and wearing the band after the first year. Broader FDA claims or major insurer adoption would strengthen Hilo even more.

Sky Labs faces a different test. Most of its disclosed use still comes from South Korea. The company must show that its clinical model can travel to Europe and Japan, where approval alone will not guarantee reimbursement or doctor adoption.

Biobeat could move into the top two by disclosing strong U.S. utilization. Thousands of completed tests, large cardiology networks or regular patch reorders would carry more weight than another financing or partnership announcement.

OUR METHODOLOGY

This analysis compares the wearable blood pressure startups with the strongest combination of working hardware, clinical evidence, regulatory progress and visible commercialization. We define the category narrowly around devices worn on the wrist, finger or chest that estimate systolic and diastolic blood pressure without repeatedly inflating a conventional arm cuff.

We assessed the companies across commercial traction, medical adoption, product maturity, regulatory position, clinical evidence, recent momentum, scalability, strategic distribution and the durability of each company's advantage. Shipped products, prescriptions, institutional use, regulatory decisions, published studies and defined distribution agreements carried more weight than preorders, planned launches or broad partnership announcements.

We did not combine unlike evidence into a mechanical score. A prescription, a device sale, an FDA clearance and a clinical study demonstrate different forms of progress, so each was interpreted within the product's intended use. The final ranking reflects the pattern across the full body of evidence rather than one headline number.

Funding totals are approximate because company disclosures and financing databases do not always separate equity, grants, debt and strategic investment in the same way. We used disclosed totals where possible and rounded estimates where the available records differed.

Regulatory status was checked against official FDA records and company disclosures for Korean approval, European certification and other market authorizations. We also distinguished clearly between regulated medical products and wellness products, especially where the same company sells related systems under different claims.

Clinical evidence was judged by the reference method, participant count, study duration, publication quality and whether the study tested stability over time rather than accuracy during a single controlled session. We also considered real-world medical use because a technically acceptable device can still fail if clinicians cannot integrate it into routine care.
