SUMMARY
The top startups in the neurotechnology market today are Neuralink, Science Corporation, Synchron, Precision Neuroscience and Cognito Therapeutics, with Neuralink still first overall because it combines the deepest capital base, a growing chronic implant cohort and the broadest general-purpose BCI ambition.
Science Corporation is the clearest reminder that the most famous BCI company is not automatically the furthest ahead on medical execution. PRIMA has already crossed into European commercial availability, putting Science ahead of Neuralink on the path from neural interface to sellable treatment.
The market is splitting around invasiveness rather than converging on one winning architecture. Neuralink and Paradromics chase bandwidth close to individual neurons, Synchron reduces surgical burden through the vasculature, Precision stays on the cortical surface, and several non-invasive companies avoid brain surgery entirely.
Human evidence is much harder to compare than headline patient counts suggest. Cognito's 673-person pivotal trial tests therapeutic efficacy at conventional clinical scale, Precision's 95-plus patients mainly show repeatable hospital use, and Neuralink's smaller cohort says more about what it means to live with a chronic BCI.
Cognito Therapeutics may have the most consequential near-term readout in the group. A strong result from its 673-person Alzheimer's study could suddenly give a non-invasive company one of the deepest efficacy datasets in private neurotechnology.
Cala Health is easy to underrate because its technology looks less futuristic. Yet prescription use, FDA clearance, Medicare coverage and growth financing put Cala much closer to an ordinary scaled medical-device business than most implant startups.
Recent funding is extremely concentrated at the high-ambition end of the market. Neuralink, Merge Labs, Science and Synchron account for roughly 94% of the six large recent financings reviewed here, showing how aggressively investors are backing platforms that could eventually support multiple neurological applications.
Merge Labs is the clearest example of funding getting ahead of evidence. Its roughly $252 million seed round makes it impossible to ignore, but almost all of the company's current rank comes from technical ambition and investor conviction rather than comparable human validation.
The biggest market may not belong to the highest-bandwidth interface. Patients with severe paralysis may accept intracortical surgery, while much larger populations with Alzheimer's disease, depression, tremor or consumer cognitive use cases will usually require something easier to implant, wear or use at home.
The ranking is unusually sensitive to a handful of upcoming milestones. Cognito needs a pivotal readout, Science needs commercial adoption, Neuralink needs repeatable scale, Precision needs chronic implantation, and Paradromics, INBRAIN and Motif still need much deeper human evidence.
Neuralink is winning the general implanted BCI race today, but the sector no longer looks like a single race with everyone else chasing second place. Different companies are already leading in commercialization, procedure simplicity, cortical recording, therapeutic scale and reimbursed wearable neurostimulation.

What should actually count as a top neurotechnology startup today?
The top neurotechnology startups today are the companies that have turned difficult neuroscience into human results, a believable path to approval, or a product people can actually use.
That immediately creates a problem with any simple ranking. Neuralink, Synchron and Paradromics are trying to decode brain activity directly. Science Corporation is restoring a limited form of vision through a retinal implant. Cognito Therapeutics is testing light-and-sound stimulation in hundreds of Alzheimer’s patients. Cala Health already sells a prescription wearable that stimulates nerves at the wrist. Neurable is trying to put EEG sensing inside ordinary headphones.
We therefore include companies whose main product records, decodes, stimulates or directly interfaces with the nervous system. Conventional CNS drug companies and generic mental-health apps fall outside the scope.
Funding alone tells us surprisingly little. Our ranking gives the most weight to human evidence, technical advantage, regulatory progress, commercial position and the size of the platform the company could eventually build.
Is Neuralink still the number-one neurotechnology startup?
Neuralink is still our number-one neurotechnology startup overall because no rival currently matches its combination of capital, chronic human implants, engineering depth and number of applications being tested.
Neuralink raised $650 million in its Series E, giving the company a financial advantage that is hard to overstate in an industry where implants, surgical robots, clinical trials and manufacturing all consume enormous amounts of capital.
The clinical program is also scaling unusually quickly for an implanted BCI. Neuralink went from three participants in 2024 to 21 reported participants worldwide by early 2026. Its own implantation timeline shows one surgery in the first half of 2024, two in the second half, four in the first half of 2025 and 13 in the second half. Almost two-thirds of the reported implantations therefore happened during a single six-month period.
People are also using the system outside tightly controlled laboratory demonstrations. Neuralink says one medical student has used Telepathy for as much as 17 hours in a day for tasks such as reading papers and completing coursework. Other participants have controlled computers and assistive robotic arms. The company currently lists active programs for digital and robotic control as well as speech restoration, with a vision-restoration trial planned next.
The unresolved part is scale. Twenty-one participants cannot tell us what durability, surgical economics or reimbursement look like across thousands of patients. Neuralink still leads because it has covered more ground than any direct rival.

Is Science Corporation actually further ahead than Neuralink?
Science Corporation is further ahead than Neuralink on commercialization because PRIMA is already authorized for sale in Europe, while Neuralink remains entirely investigational.
PRIMA treats central vision loss caused by geographic atrophy from age-related macular degeneration. The system uses a tiny photovoltaic implant placed beneath the retina together with specialized glasses that project visual information onto the implant.
The pivotal European study included 38 patients at 17 centers across five countries. The results published in The New England Journal of Medicine were unusually tangible: patients improved by an average of 25.5 letters on an eye chart, 80% improved by at least 10 letters after 12 months, and 84% could use the prosthetic vision to read letters, numbers or words.
Science then crossed the regulatory line that most new BCI companies are still approaching. PRIMA received a CE mark authorizing commercial availability across 30 European countries. The company has begun its European launch, with Germany expected to host the first commercial implant and reimbursement work underway elsewhere.
Science also raised $230 million in 2026, bringing total capital invested since its founding to approximately $490 million. That gives it enough money to commercialize PRIMA while continuing to develop more experimental programs such as Biohybrid neural interfaces.
If we ranked purely on medical execution today, Science would be number one. Neuralink still has the broader and more ambitious general-purpose cortical platform.
Which less-invasive BCI challenger is stronger today: Synchron or Precision Neuroscience?
Synchron has the stronger case for scalable implantation, while Precision Neuroscience currently has the stronger case for high-resolution cortical recording without penetrating brain tissue.
Synchron delivers its Stentrode through the jugular vein and places the recording device inside a blood vessel near the motor cortex. The company says the clinical procedure takes roughly two hours and that most participants go home the following day. That gives Synchron a very different scaling path from Neuralink, which relies on a robotic system to insert electrode threads directly into the cortex.
Synchron already has longer human history than most current BCI startups. Its earlier U.S. COMMAND study followed six participants, and people have now lived with its implant for several years. The company currently lists two active clinical studies and raised $200 million in late 2025 to finance clinical expansion, commercialization work and its next generation of technology.
Precision takes the opposite route. Its Layer 7 array contains 1,024 electrodes on an extremely thin film placed directly on the cortical surface. The company currently reports more than 95 implanted clinical-study patients and partnerships with more than 15 hospitals. Researchers have already used Layer 7 for real-time decoding of movement and speech and for computer and robotic control.
Precision also holds FDA 510(k) clearance for its cortical interface, although that clearance covers temporary clinical recording, monitoring and stimulation rather than its future permanent wireless BCI.
The trade-off is straightforward. Synchron sacrifices some access to individual-neuron activity in exchange for a catheter-based procedure. Precision gets much closer to the cortex and captures richer signals while still avoiding penetrating electrodes. Today, Synchron looks easier to scale procedurally; Precision looks stronger if signal quality and limited tissue damage are the priority.

Which emerging implanted BCI challengers matter most: Paradromics or INBRAIN?
Paradromics and INBRAIN have both moved into meaningful human testing, but Paradromics is the stronger high-bandwidth BCI challenger while INBRAIN is making the more unusual materials bet.
Paradromics completed the first implant in its FDA-authorized Connect-One early feasibility study in June 2026 at the University of Michigan. The company has deliberately optimized Connexus for high neural data rates, using a dense intracortical microelectrode array to collect signals close to individual neurons.
That makes Paradromics particularly interesting for speech restoration, where large amounts of neural information have to be decoded quickly. The registered study is still tiny, with an estimated enrollment of two participants, so human proof remains limited. Neuralink already has a much larger implant cohort and is also running a speech-restoration program.
INBRAIN uses a very different architecture. Its ultrathin graphene arrays can record neural activity and deliver stimulation, with up to 1,024 electrodes on a surface smaller than a fingertip.
The company has completed its first human study. Ten people were recruited during brain-tumor surgery and eight were treated with the graphene interface. Researchers reported no device failures during use and no device-related adverse events through surgical discharge. In awake procedures, the system also recorded neural activity while patients performed tasks such as naming objects.
Paradromics now needs to prove that its high-bandwidth architecture creates a measurable advantage in speech or control. INBRAIN needs to show that graphene provides a real long-term therapeutic advantage rather than simply excellent acute recordings. Both have crossed the line from promising architecture into clinical testing, but neither has enough human evidence yet to challenge the top four.
Is Cognito Therapeutics further ahead than most brain-chip startups?
Cognito Therapeutics is much further ahead in clinical scale than almost every brain-chip startup because its pivotal Alzheimer’s study has already enrolled 673 people.
Cognito's Spectris system uses synchronized light and sound to stimulate gamma-frequency brain activity. Patients use the device at home for roughly an hour a day, so the company is attacking neurotechnology from the opposite direction of implanted BCI companies.
Its earlier OVERTURE trial enrolled 76 patients with mild-to-moderate Alzheimer's disease. Cognito reported a 77% reduction in decline on an activities-of-daily-living measure and a 69% slowing of brain-volume loss after six months compared with control. Those results were strong enough to justify a much larger test, but a 76-person feasibility study can still produce effects that disappear when the sample becomes larger.
The HOPE pivotal trial is fully enrolled with 673 participants across roughly 70 U.S. sites. Across Cognito's studies, patients have already completed more than 100,000 treatment sessions.
That scale is why Cognito sits in our top five. If the pivotal trial confirms a meaningful treatment effect, its clinical evidence would be deeper than anything currently available from the headline implant startups.

Is Cala Health already a real neurotechnology business?
Cala Health is already much closer to a normal medical-device business than most neurotechnology startups because doctors can prescribe its FDA-cleared therapy and Medicare covers qualifying essential-tremor patients.
Cala's wrist-worn system measures a person's tremor and delivers individualized electrical stimulation through peripheral nerves. The therapy targets essential tremor and Parkinson's-related hand tremor without putting hardware anywhere near the brain.
The company has built evidence over several years rather than around one dramatic clinical milestone. It has run randomized trials, published real-world studies and expanded from essential tremor into Parkinson's disease. In April 2026, Cala received FDA clearance for its next-generation TAPS wearable.
Five days later, Trinity Capital announced $50 million in growth capital for Cala specifically to support commercial expansion. Cala is now financing adoption rather than proving from scratch that the device works.
Its platform is narrower than Neuralink's or Science's, but Cala has already reached prescriptions, reimbursement and repeat home use.
Could Motif Neurotech make brain implants practical for depression?
Motif Neurotech is one of the most interesting early therapeutic neurotechnology companies because its tiny implant could sit between external brain stimulation and much more invasive deep-brain surgery.
Motif's device is designed to sit in the skull above the dura, without entering brain tissue. It is wirelessly powered and stimulates cortical networks linked to treatment-resistant depression.
The FDA authorized Motif to begin its first clinical trial in 2026. The registered RESONATE study plans to evaluate safety, tolerability and preliminary efficacy in 20 people with treatment-resistant depression. The trial registry still listed the study as not yet recruiting at its latest verification, so human efficacy data from the intended system do not exist yet.
That keeps Motif well below Cognito, Cala and the established BCI companies for now. Its next ranking move depends almost entirely on what happens in those first patients.

Can Neurable make consumer neurotechnology genuinely useful?
Neurable currently has the clearest consumer-neurotechnology strategy because it is trying to hide EEG inside products people already wear instead of asking consumers to buy a dedicated brain-sensing device.
The company combines compact EEG sensors with software that estimates states such as focus, mental fatigue and cognitive recovery. Its technology has already appeared in Master & Dynamic headphones, and Neurable is now pushing a broader licensing model for manufacturers of headphones, glasses, hats and other wearables.
That shift changes the size of the opportunity. Selling a specialist $500 neurotech headset creates a small category. Licensing brain sensing into products with existing distribution could create something much larger.
Neurable raised $35 million in a Series A in late 2025, bringing total funding to roughly $65 million. A few months later, it publicly expanded the licensing strategy rather than focusing only on its own branded hardware.
The open question is whether consumers care enough about EEG-derived fatigue, focus or cognitive-state data to use it repeatedly. Until that is clear, Neurable remains more promising as an embedded technology supplier than as a proven consumer platform.
Is Merge Labs already a top neurotechnology startup, or mostly a huge funding round?
Merge Labs is already important enough to track closely, but its $252 million seed round currently tells us much more about investor ambition than about clinical leadership.
The company emerged with one of the largest seed financings ever seen in neurotechnology. OpenAI participated and said it would collaborate with Merge on scientific foundation models and other AI tools. Reporting around the financing put the total seed round at roughly $252 million and the valuation around $850 million.
Merge is pursuing a very different BCI architecture. Its research combines biology, devices, AI and techniques including ultrasound, with the long-term goal of interacting with the brain at high bandwidth without relying on conventional electrode implants.
The prize would be enormous if it works because every current BCI architecture compromises between signal quality and invasiveness.
For now, though, Merge has almost no comparable human evidence. Its funding and technical ambition put it on the list; its clinical position keeps it near the bottom of the current ranking.

Which neurotechnology startups have the strongest human evidence right now?
Science Corporation, Cognito Therapeutics and Cala Health currently have the strongest human evidence for products close to real clinical use, while Neuralink and Precision lead the newer BCI companies on different kinds of human experience.
The numbers need context because patient counts measure different things. Cognito's 673-person randomized Alzheimer’s study tests whether a treatment changes disease outcomes. Science's 38-patient pivotal PRIMA study supported European approval. Precision's 95-plus patients mostly demonstrate that its cortical interface can be used repeatedly in clinical settings. Neuralink's smaller cohort tells us more about what happens when people actually live with a permanent BCI.
Synchron adds something patient counts capture poorly: duration. Some Synchron participants have lived with an endovascular BCI for years. Paradromics and INBRAIN have more recent human milestones but remain at much smaller scale.
Raw patient counts cannot decide the ranking on their own. The useful comparison is what each dataset has actually proved.

Where is neurotechnology funding concentrating now?
Recent neurotechnology financing is heavily concentrated in a few BCI platforms, with Neuralink, Merge Labs, Science Corporation and Synchron absorbing most of the biggest disclosed rounds.
We looked at six large financing events announced from 2025 through today among the companies in our shortlist: Neuralink's $650 million Series E, Merge's roughly $252 million seed round, Science's $230 million Series C, Synchron's $200 million Series D, Cala's $50 million growth-capital commitment and Neurable's $35 million Series A.
Together, those six events represent roughly $1.42 billion. The four BCI-heavy companies at the top account for about $1.33 billion, or 94% of the total.
That concentration shows where investors are placing the most aggressive long-term bets. Cala is a useful contrast: its $50 million is financing commercial expansion rather than the basic development of an unproven platform.

Which neurotechnology approach could actually reach the most people?
The neurotechnology architecture with the highest theoretical bandwidth is unlikely to be the architecture that reaches the most people, so we expect several winners rather than one universal BCI platform.
Neuralink and Paradromics sit at the high-bandwidth end. Their electrodes operate close to individual neurons, which is useful for rich motor control, speech decoding and eventually more complex interfaces. Surgery and long-term tissue interaction become harder as the hardware gets closer to the neurons.
Synchron, Precision and the non-invasive companies make different compromises. Synchron lowers the surgical burden with a catheter-delivered implant. Precision stays on the cortical surface. Cognito can treat patients through a headset at home, Cala uses a wrist wearable, and Neurable wants EEG to disappear inside consumer devices people already own.
That changes the size of the addressable population. People with severe paralysis may accept an intracortical implant if it restores communication. Millions of people with Alzheimer's disease, depression or tremor will usually require a much lower procedural burden.
The biggest neurotechnology market may therefore come from a less spectacular device that is simply easier to use at scale.

Who is actually winning each part of the neurotechnology market?
No single neurotechnology startup currently wins every category; Neuralink leads general implanted BCI development, Science leads commercial neural interfaces, and several other companies have clearer advantages in specific architectures or applications.
That fragmentation looks rational rather than temporary. Restoring speech, treating Alzheimer's disease, controlling a computer and tracking cognitive fatigue require very different products.

What could change this neurotechnology ranking fastest?
The neurotechnology ranking could move dramatically with just a few clinical results because several important companies are sitting at very different validation thresholds.
Cognito has the most binary near-term test. Its earlier 76-person study produced large reported effects, while the pivotal study includes 673 participants. A strong pivotal result would give Cognito one of the deepest clinical datasets in private neurotechnology. A disappointing result would erase much of the advantage created by its trial scale.
Science has already cleared the efficacy and European regulatory hurdles for PRIMA. Its next test is commercial: how quickly hospitals adopt the system, whether reimbursement follows, how many patients actually receive implants and whether treatment economics work outside a controlled trial.
For Neuralink, the next proof point is repetition. Going from 21 participants to a much larger cohort while maintaining device reliability, strong performance and low serious complication rates would make the company considerably harder to catch.
Precision needs to turn temporary human use into chronic implantation. Paradromics needs to show an advantage in speech. INBRAIN needs long-term therapeutic evidence. Motif needs its first treated depression patients. Merge needs convincing human data.
Those milestones will tell us far more than another large funding round.
So what are the top neurotechnology startups today?
The top neurotechnology startups today are Neuralink, Science Corporation, Synchron, Precision Neuroscience and Cognito Therapeutics, with Neuralink still first overall and Science now much closer than the headline ranking suggests.
Neuralink remains our number-one company because it combines the largest financial resources with a rapidly expanding chronic implant program, an integrated surgical system and several potential applications.
Science deserves second place after PRIMA's European approval and commercial launch. Among the startups we reviewed, Science has done the most complete job of turning a neural-interface technology into something regulators will allow doctors to sell to patients. The broader Biohybrid program also gives the company upside beyond retinal implants.
Synchron takes third place. We think the endovascular approach could prove extremely important if motor-control BCIs become a real medical market because procedure complexity may eventually matter as much as neural bandwidth.
Precision ranks fourth, very close behind Synchron. Its 95-plus clinical-study patients give it unusually broad human exposure, while its non-penetrating cortical approach looks like one of the more credible compromises between signal quality and invasiveness.
Cognito comes fifth. A 673-person pivotal Alzheimer’s trial gives the company a level of clinical maturity that most BCI startups simply do not have. Its ranking could move sharply once the pivotal outcome is known.
Paradromics and INBRAIN come next because both have now crossed into meaningful human testing with differentiated architectures. Cala follows with far stronger commercial proof than most of the list but a narrower technological platform. Merge remains one of the highest-upside bets in the sector, although the gap between its funding and its human evidence is currently enormous. Motif and Neurable complete our top group through two very different opportunities in psychiatric stimulation and consumer brain sensing.
Neuralink is winning the general-purpose implanted BCI race today. Science is winning the race to commercialize a new neural interface. Cognito has reached the largest therapeutic trial. Cala already operates a reimbursed neurotechnology business. Synchron may have the easiest invasive procedure to scale.
Neuralink is still our top startup overall, but there is no longer a convincing case for treating the rest of neurotechnology as a race for second place.

This analysis ranks the top neurotechnology startups by asking which companies have converted difficult neuroscience into the strongest combination of human evidence, technical progress, regulatory advancement, commercial execution and platform potential. We include companies whose main product records, decodes, stimulates or directly interfaces with the nervous system. That includes implanted BCIs, retinal neural interfaces, peripheral neuromodulation, non-invasive brain stimulation and consumer EEG. Conventional CNS drug developers and generic mental-health apps are outside the scope.
Human evidence carries the most weight, but we treat different forms of evidence differently. A 673-person randomized therapeutic trial, years of chronic implant use, repeated temporary cortical recording in hospitals and a 38-patient pivotal study that supports commercial authorization are not interchangeable achievements, so we assess what each dataset actually proves.
Regulatory progress is interpreted just as narrowly. Science Corporation's CE mark matters because PRIMA can enter commercial use in Europe. Precision Neuroscience's FDA 510(k) clearance matters for temporary cortical recording, monitoring and stimulation, but we do not treat it as approval of Precision's future permanent wireless BCI. FDA authorization to begin a study is treated as permission to test, not as evidence that a therapy works.
We also compare companies inside the architecture they are actually pursuing. Intracortical, endovascular, cortical-surface, retinal, peripheral and non-invasive systems make different trade-offs between bandwidth, invasiveness, procedure complexity and addressable population. The ranking therefore rewards technical advantage when it is backed by a credible clinical or commercial path, rather than assuming the highest-bandwidth approach is automatically best.
Recent funding helps us understand which platforms investors are willing to finance through expensive clinical and manufacturing programs, but capital does not outweigh human proof. This is especially important for companies such as Merge Labs, where the financing is already exceptional while comparable clinical evidence remains very early.
